COVID-19 VACCINE UPDATE – Eligible Populations

Governor DeSantis issued Executive Order 21-47 (amending Executive Order 20-315) expanding the populations eligible to receive COVID-19 vaccines.  If you are eligible to receive the vaccine and require a form, we are happy to help complete the required form.  Please schedule an appointment today by calling Celebration Family Physicians 407-566-8898.
Populations Eligible to Receive COVID-19 Vaccines:
  • Long-term care facility residents and staff
  • Persons 65 years of age and older
  • Health care personnel with direct patient contact
  • K-12 school employees
  • Sworn law enforcement officers 50 years and older
  • Firefighters 50 years and older
  • Persons under the age 65 deemed extremely vulnerable to COVID-19 (form required to be completed and signed by a physician)**
**Clinically vulnerable patients are under the age of 65 with any of the following conditions:
  • Lung Conditions
    • Severe Asthma: if without treatment, symptoms occur throughout the day and severely limit daily physical activities, nighttime symptoms occur often (sometimes every night), and lung functions tests are abnormal.
    • COPD (Including Emphysema)
    • Cystic Fibrosis
  • Heart Conditions
    • Heart failure with ejection fraction less than 40%
    • Coronary Artery Disease
    • Cardiomyopathy
    • Complex Adult Congenital Heart Disease
  • Kidney conditions
    • End Stage Renal Disease on Dialysis
    • Chronic Kidney Disease
  • Immunocompromised State (Weakened Immune System)
    • Cancer on Active treatment. AIDS with an absolute CD4 count equal or less than 200 within the past six months
    • Solid Organ Transplant
    • Blood or Bone Marrow Transplant
    • Autoimmune Disease on Immunosuppressive Medication
  • Severe obesity (body mass index [BMI] >40)
  • Diabetes
  • Sickle Cell Disease
  • Down Syndrome

 

 

Emergency Use Authorization of Johnson & Johnson’s Janssen COVID-19 Vaccine:
The Federal Food and Drug Administration (FDA) has issued an Emergency Use Authorization to permit the emergency use of the product Janssen COVID-19 Vaccine for individuals 18 years of age or older.

Celebration Family Physicians

Drs. Craig and Johanna Yunk are Board-Certified Physicians who are both deeply rooted in ‘family’ – raising their family of four boys (all under the age of 9) and specializing in Family Medicine.  Craig and Johanna’s inspirational journey began in Medical School at St. George’s University where they met.  Since then, they have been inseparable – they continued to complete their residency training together at Advent Health (then Florida Hospital) and following residency, they continued to be part of the Advent Health Team for many years.  However, the couple had aspirations to connect even more with their community.  They wanted to open their own family practice office close to their home and after years of planning and collaboration, the couple’s aspirations were realized when Celebration Family Physicians opened their doors on August 2020.  Their family practice is now open for their community.  Walking through their doors, you can expect to receive exceptional care from a couple who truly have a deep passion for wellness.

ABOUT DR. CRAIG YUNK

Craig Yunk, MD, is the medical director and founding member of Celebration Family Physicians. He specializes in Family Medicine, with a special interest in pediatric care and sports medicine.

Raised in Manhattan, Kansas, Dr. Yunk is a Kansas State Wildcat fan.  He graduated from KSU with scholarships, and from St. George’s University Medical School. He completed his residency at Florida Hospital, where he was awarded Resident of the Year and was also Chief Resident. Dr. Yunk helped establish AdventHealth’s Family Medicine of Celebration and served on the Executive Board for AdventHealth Medical Group. Currently, he is AdventHealth’s Physician Network Regional Leader. Dr. Yunk also serves as a faculty preceptor to Florida State University medical students.

ABOUT DR. JOHANNA GULMATICO YUNK

Johanna Yunk, MD is the co-founding member and manager of Celebration Family Physicians.   She specializes in Family Medicine, with a special interest in hospital medicine and administration.

Dr Yunk was born in the Philippines and raised in New York City.  She attended Emory University where she graduated with a Bachelors Degree in Neuroscience and Behavioral Biology.  She graduated medical school with Honors from St. George’s University.  After completing residency at Florida Hospital, Dr Yunk served as a hospitalist at the Advent Health Celebration and Apopka Campuses.  She has held several leadership positions over the years and currently the Department of Medicine Chairman and Director of the Surgical Unit for the Apopka Campus.
